There is a commitment to the protection of your personal information;

The collection, use, retention and disclosure of your personal information is limited to that which is reasonable to achieve the purposes of providing physical therapy treatment;

Information is only disclosed to third parties for the specific purposes identified, with your express consent or as otherwise permitted by law;

Physical safeguards are in place to secure your personal information;

There is a mechanism to access your own personal information, and consideration will be given to requests for changes to the personal information where accuracy of the information is questioned;

A Privacy Officer is available so that questions or concerns regarding the Privacy Policy and/or procedures or the personal information can be answered; and

There is a process to ensure the legislation is being properly enforced.
